______F - FINANCIAL EVIDENCE:______________________________________
A review of the I-864, I-864EZ, I-864W or I-864A (if applicable) Affidavit of Support form and
supporting documents that you submitted has revealed that you have provided incomplete information in
your response or you have not responded to some questions.
Please pay specific attention to the questions marked in the attached I-864/A checklist. You will need to
complete and submit a new form with the changes noted on the attached checklist.
You will need to follow the instructions for the Affidavit of Support (I-864) that are posted on the
Department of State website at the following address:
www.ImmigrantVisas.state.gov. From there, you can fill out a new form online, print the new
completed form and mail it to NVC. If you are the Petitioner, please read all instructions and complete
the correct form to submit to NVC. An Affidavit of Support form is required to be submitted from the
Petitioner on each case regardless of his/her income level.
NVC will hold this case until you send us these needed items. When you return the documents to NVC,
please make sure that the documents submitted are in the correct order and stapled together, as stated in
the I-864 Important Notice instructions.

Required for immigrant visa applicants only.
For Vietnamese residents, non-Vietnamese residents, and foreigners currently residing in Vietnam:
Requests for a “Justice of Record Check #2” (Phieu Ly Lich Tu Phap So 2”) must be made at the
Department of Justice office located in the district where the applicant currently resides, or at the
applicant’s official residence. The official residence is registered in the “household registry” (Ho Khau)
issued by the district police. The processing fee to request this document is 200.000VND per applicant,
and the record check takes approximately 10 working days to complete. The applicant cannot grant
authority to someone else to request the “Justice Record Check” on his/her behalf.
The applicant should be prepared to present (2) sets of the following documents at the time of therequest:
• Completed application form (Form 03/TT-LLTP.) Please check the box in Justice Record
Check #2;
• Applicant’s National Identification Card or passport;
• Proof of residence location and length of time the applicant has resided in Vietnam, such as
the household registry book (Ho Khau), the temporary residence registry book, the permanent residence
card or a residence certification from the local People’s Committee.
For foreigners who formerly resided in Vietnam and no longer reside in Vietnam

Khi người bảo trợ không đủ income để bảo trợ tài chính , phải dùng đến các giá trị tài sãn khác , để bù vào khoãng thiếu hụt incom .THÌ GIÁ TRỊ TÀI SÃN NÀY PHẢI GẤP 5 LẦN SỐ INCOM CÒN THIẾU .(diện vợ chồng thì gấp 3 lần ).
3 PHIẾU LÝ LỊCH TƯ PHÁP SỐ 2 .
Trong trường hợp người bảo lảnh , người đồng tài trợ joint sponsor không đủ incom để bảo trợ tài chính cho gia đình bạn , thì nên tìm kiếm một đồng tài trợ thứ 2 .một hồ sơ bảo lảnh có quyền dùng đến 2 người đồng bảo trợ .
theo tôi được biết , trường hợp dùng tài sãn giá trị để bù đắp cho khoãn thiếu hụt income , sẽ không được LSQ HOA KỲ chấp nhận khi phỏng vấn , dù điều này có được ghi trong hướng dẫn .
Có việc làm và luôn có thu nhập , đóng thuế đầy đủ là những yếu tố quan trọng để hồ sơ bảo trợ tài chính được chấp nhận .
