Please submit this applicant's original or a certified copy of the original document listed above. Each

original registration of birth.
The certificate must contain the:
• Person’sdate of birth
• Person’s place of birth
• Names of both parents, and
• Annotation by the appropriate authority indicating that it is an extract from the official records
If the certificate is a certified copy of the original, it must come from the issuing government authority
and it must contain annotation by the issuing government authority indicating that it is an extract from
the official records.
Documents must be from the appropriate issuing government authority. Failure to providethe
correct document will delay processing of your case and may result in re-scheduling of your
interview, or refusal of a visa.
In addition to sending the original documents or certified copies of the documents to the NVC, a
photocopy of the original documents must be submitted.
Unobtainable birth certificates
If the applicant’s birth record is unobtainable, please obtain a certified statement from the appropriate
government authority stating the reason the applicant’s birth record is not available. With the certified
statement the applicant must submit secondary evidence. For example:
• A baptismal certificate that contains the date and place of birth, as well as both parents names
(providing the baptism took place shortly after birth)
• An adoption decree for an adopted child
• An affidavit from a close relative, preferably the applicant’s mother, stating the date and place of
birth, both parents names, and the mother’s maiden name.
Suitability of documentation for immigration purposes
For many countries, the Department of State has provided country-specific criteria for ensuring that
applicants submit visa processing appropriate documents. Please follow the instructions below to
determine if the Department of State has issued any country-specific guidelines for the requested
document.

the "household registry" (Ho Khau) issued by the district police. There is a fee. The record check takes
approximately three weeks to complete. The record check must be initiated in the district listed in the
household registry as the citizen's official residence.
Non-Residents: Vietnamese citizens living overseas may request the "Justice Record Check" through
the Vietnamese Embassy in their current country of residence. They may also request the "Justice Record
Check" in person at their last place of residence in Vietnam, as reflected in their household registry.
Foreigners: Foreigners who reside or resided in Vietnam should contact the Vietnamese Department of
Immigration in the district in which they currently reside and/or formerly resided to request the "Justice

Chuẩn bị những giấy tờ sau:

2/ Khai sinh (bản chính hoặc bản sao được xác nhận bởi nơi cấp bản chính)
3/ Lý lịch tư pháp (bản chính)
4/ 2 tấm hình Visa (chụp đúng chuẩn đi Hoa Kỳ, ngoài tiệm hình họ sẽ biết)
Tất cả giấy tờ trên phải ghi Case number lên góc trên bên phải của mỗi tờ giấy.
Hình Visa nhớ ghi tên họ ngày tháng năm sinh sau lưng tấm hình.
Bộ hồ sơ sau khi đầy đủ nhớ in tờ Document Cover Sheet có mã vạch kẹp lên trên bộ hồ sơ, đóng thành 1 gói, ra bưu điện gửi bảo đảm đến địa chỉ NVC có ghi trong checklist.
@ nên chuẩn bị thật đầy đủ theo yêu cầu và gửi trực tiếp cho NVC, không cần phải gửi cho luật sư mất thời gian. Chúc thành công !

. Mà có giữ thì hỏi đọc số invoice chắc cũng không biết nó là gì ấy .

Document cover Sheet là tờ có trong cheklist mà NVC đã gửi email cho bạn, xem kỹ lại các trang trong checklist, chắc chắn sẽ có tờ này, trong đó có cả địa chỉ của NVC.

Vậy anh có thể cho em biết có cần bản dịch ra tiếng Anh để nộp không anh ? Em vẫn đang gấp rút làm LLTP mà 1 tuần mới có híc , nộp luôn cái giấy độc thân.

Họ thông báo là họ đang khám xét lại HS của bạn coi có được hưởng luật CSPA khg, vì vậy bạn nên chờ và họ sẽ gởi thư về trả lời cho bạn. Nếu bạn liên lạc với LSQ thì họ cũng không thể nào cho bạn biết thêm nhiều hơn được nữa. Vậy thì bạn chịu khó chờ nha. Mình thấy có nhiều người chờ xét CSPA từ 30 đến 90 ngày đó, tuỳ vào thời điểm visa ít hay nhiều. Họ có ghi rõ là "no timeframe" (không thời hạn nhất định), tức là có nhanh hoặc chậm tuỳ. Diện F2A thì thường thường đến ngày nhận giấy pv mà bạn khỏang 21 tuổi 8 tháng thì bạn có hy vọng cao lắm. Còn vượt qua nữa thì có lẽ HS bạn sẽ tụt xuống F2B (chờ thêm khỏang 4-6 năm nữa)
Theo mình biết rõ thì thời điểm này hơi chậm chút xíu nghe bạn vì Xmas và New Year. Nhiều người Mỹ nghĩ holiday + vacation cả 2 tuần lễ, và bắt đầu năm mới vào ngày 3/1/2012.
Đừng lo lắng nhiều quá, HS bạn khg bị thất lạc đâu.
